**Below you will find links containing instructions to set up and complete the online medical forms required by the Sports Medicine Department. Student-athletes will not be cleared for participation until these forms are completed.
*Deadline to complete these forms is July 15, 2025 for Fall Sports athletes.
*The Deadline for Winter/Spring sports will be August 1, 2025.
