Dear Battling Bishop Student-Athletes and Parents,

**Below you will find links containing instructions to set up and complete the online medical forms required by the Sports Medicine Department. Student-athletes will not be cleared for participation until these forms are completed.

*Deadline to complete these forms is July 15, 2026 for Fall Sports athletes.

*The Deadline for Winter/Spring sports will be August 1, 2026.

NEWCOMERS- Please review the information below.

Welcome to the Battling Bishop Family!

This checklist is for our newcomers—Please review and complete the required pre-participation medical screening steps below prior to your arrival on campus to ensure you are cleared for intercollegiate athletics at North Carolina Wesleyan University. Please note that this process is separate than the paperwork required to be sent in to Health Services, and most of the requirements should be completed prior to arrival on campus.

	Create an account with our Electronic Medical Record System ATS. Detailed instructions are available HERE. Please read the instructions in their entirety.
	Get a Sports Physical - we request you use our physical form, needs to be completed within 6 months of start of participation in athletics at NCWU
	Obtain a copy of your Sickle Cell Test Results, commonly referred to as Hemoglobin Test Solubility.
	Take the Online Concussion Test(CNT), details on how to access found in the instructions above. Additionally you will complete a second baseline concussion test (SCAT6) on campus after you arrive.
	Review and Sign the provided forms within ATS {Concussion Fact Sheet, Concussion Statement, Drug Testing and Supplement Information Sheet, Insurance Information Form, NCWU Secondary Insurance Information Form, Newcomer Medical History, Sickle Cell Information, Helmet Release Waiver (Football Only), Pregnancy Statement (Female Student Athletes Only), Medical Treatment Authorization for Minors (Only if you are currently under the age of 18)}
	Please review the forms mentioned above carefully, and provide ALL requested information.
	Upload your Insurance Information (as part of making the ATS Account)

	In addition to the requirements of the medical screening, there will be Institutional and NCAA forms which will need completed as well. We anticipate these being launched July 15^th via Front Rush

After you arrive on campus, you will meet with your Athletic Trainer to ensure all steps listed above are completed. If these steps are not completed, there will be a delay in participation in team activities.
